Welcome to the NHS Report a Fault tool

This service is being trialed with PFI Trusts, to enable them to report faults digitally.

It consists of 2 small websites that can run on a phone, tablet or desktop computer.

The first is for hospital staff to report a fault. It’s quick to use, with most faults being reported in under a minute, and has the option to include a photo of the problem. No username or password is required, just a valid hospital email address.

The second is a dashboard for helpdesk operators. When a fault is reported, the operator can see the report, add to their FM system, and provide a job number or comment back to the reporter.

Any PFI Trust may use the service while it’s in it’s beta phase - please contact James Green at the DHSC for details. Getting set up takes just 10-15 minutes.
